I just installed, went to the setup, and slapped on the shortcode – Tested and it works, pretty easy.
The reason it’s not getting five stars is because its sloppy – the email address box is way too short, and the text on the “Add a new Ticket button is off center and runs out of the right side of the box. We can’t put this on a clients site, so we’ll need to edit…
