Select field from drop down?

Hello everyone,

Here’s what I need to do:

I want to create a pw-protected page, and on that page have a list of names. Next to each name, I want to have a drop down where someone with access to the page can choose of the options in the drop down next to a name. Once the option is chosen, it locks the dropdown and no other person can access it, except a site admin. Here’s an example:

Name 1 [drop down]
Name 2 [drop down]
Name 3 [drop down]

Is there a plugin or something else I can use to accomplish this?
