Selected Required Drop Down Menu Items Not Recognized Upon Submission

Relative newbie to SalesForce and certainly so for this plugin. My apologies if this is a silly question; I’m probably not seeing the obvioius. Thanks for your help in advance.

Here’s the issue: “required” item fro “select(picklist)” is not recognized upon form submission, returning error “this field required.”

Field: monthly sales
Operations: x Enabled x Required
Type: select(picklist)
Label: Monthly Gross Sales
Value: Choose one
Options: Select One:|$1k-$10k|$10k-$25k|$25k-$50k|$50k-$100k|$100k-$250k|$250k +
