Maybe you did not add the mail-tag in “File attachments” field.

This is my code at form tab:
[file AttachFile limit:5mb filetypes:jpg|jpeg|png|gif|pdf|txt|rtf]

And this is at mail tab:
From: [your-name] <[your-email]>
Subject: [your-subject]

Message Body:
[your-message]

Obviously you did not add the mail-tag in “File attachments” field.
