Setting page blank / Registration Form Not Loading

When people try to register on our site (https://www.berkeyavenue.org/resources/berkey-avenue-online-directory/), the page goes blank; there is no form. When I sent to the the setting page of the plugin to see what the problem was, it too was blank. Nothing loads.
