SEVERAL forms showing on one page

I have it set where a form should embed onto one of my pages. BUT there are two embedded. I’ve tried fixing it several times with no success. Sometimes when I try fixing it there’s even four forms that are embedded on a single page.
How do I fix this? I only want ONE form embedded.
