Sharing user videos on my site… Please Help!

I need a way of allowing my site users to share their videos on my site without having to register. Anyone know how to do this?

Ideally I’d like them to fill in a form that says:
– Film Title
– Add Video (Film location: youtube etc…)
– Catagory (from drop down menu)
– Your Name

Then it goes into a library on my site and lets other users rate, comment and so on.
