Shipping address not an option

Hi,

I have an issue with the checkout page, there is no option to enter a shipping address, only a billing address.
I have viewed numerous other threads about this issue and the solutions seem to be the setting on the Shipping Options page and ensuring there are Shipping Zones.
I have checked this and setup is correct but the shipping options are still not showing.

Any ideas?
