Shortcode doesn't work

Yes but as i mentioned before now when im trying to make new form or even put the same again somewhere else it doesn’t show. Moreover the one you linked was made like that:
1. I insterted shortcode.
2. Copied code i get from shortcode.
3. Insterted code on seperate page.

Anyway now when i put shortcode in page/post etc. nothing shows up.

Try switching to the default theme and deactivating all other plugins.

when i switch to another theme then shortcode works. Obviously it’s something with my theme. Any idea what it can be?

Ok i found out that if i use <?php the_excerpt(); ?> shortcodes doesn’t work. Any clue how to fix that issue?

I’m having a similar issue. I add the shortcode to a page and when that page is viewed all that appears is the shortcode. The plugin is active. I have tried adding the shortcode in multiple areas with the same result. Any suggestions would be appreciated. Thanks.
