Shortcode non-usable after publish

Hi,

in preview mode, everything worked fine, but as soon as I publish the post the plugin becomes inactive (also, the thanks message is already displayed initially). Thus, there must be some kind of big bug.

Could you please have a look (see at the bottom of my linked website), as I think this is the best free plugin and I really wanna use it!
