Show items added via “WooCommerce Checkout Add-Ons” on packing slip

You should be able to do it querying the order and printing that data withing an action hook: https://docs.wpovernight.com/woocommerce-pdf-invoices-packing-slips/pdf-template-action-hooks/.

A more simple way would be with our Premium Templates extension and his customizer.

I used this snippet to show a cutom text in case the customer set the checkbox:

add_action( 'wpo_wcpdf_after_order_details', 'wpo_wcpdf_custom_text', 10, 2 );
function wpo_wcpdf_custom_text ($template_type, $order) {
  	$fees = $order->get_fees();
  	foreach($fees as $fee) {
	  if (strpos($fee['name'], 'postcard') !== false) {
		echo('Text to be shown on packing slip goes here');
	  }
	}
}
