Show the page the form was send from

Hi,
I would like to show in my form from which page the visitor came before he filled in the form. I have many pages that show a contact button. this button leads to the contact form. But instead of making a new form for every page, it would be much handier to have a field that shows the page (title) of the page where he clicked the contact form. Am i clear on what I mean?
Ex. I am on page A, there is a contact button that I click. The contact form wil show a field that says ” I m interested in [page A], please call me back.”
