    If I remember correctly the :before and :after pseudo selectors do not work on img tags. You would probably have to use some JQuery/JavaScript if you do not want to hack the plugin.

    Descriptions & titles can be added since version 1.4. You need to create a template part in your theme (parts/wp-instagram-widget.php) which will allow you to override the output of the plugin completely. Let me know if you have questions.
