Sidebar Login in Nafo theme

I added the Sidebar Login plug in to a Nafo theme. I can’t get it to show. I tried it on another theme to see if that was it. Still nothing. How can I get this plug in to work. Locally hosted and uploaded to a test site. treesvote.com Did I miss a step?
