Sidebar Login issue

I am using the SIDEBAR LOGIN plug-in for a site I have on a test server. So it’s at “www.mysite.com/testsite”

You can log in alright, but when you go to log out you get this error.

First this while the page title is called, “WordPress Failure Notice”::
You are attempting to log out of Infinity Instruments of La Crosse, Wisconsin
Do you really want to log out?

You click Yes, and then it takes you to the WordPress WP-Login page.

I have it set where it should just take you back to the home page of the site. So if I close out the window, and go back to the site, I’m still logged in?? So technically it’s not logging me out. Any idea why this may be doing it?? Is it behaving like this due to how I have the install of WordPress on a subfolder? Very frustrated, and confused. Thank you!!
