Single Sign On Failed.

After following the instructions in the video the page I am redirected to has the url:
https://www.example.com/?auth=sso&code=wfq8p5cqf4yqxtsqyrymunhsdvosul87fgdh3tq1k
and contains the plain text:
Single Sign On Failed.
If I was logged out I then go to the server site, I am logged in as the account I used to authorized but no new user has been created on the client site. What can I do to correct this?
