There are two possible causes of this, the first is that the theme is not using the proper hook for the title. Check the header.php file of your theme and see what it has where you see the <title> tag. Most themes use wp_title() – https://codex.www.ads-software.com/Function_Reference/wp_title

Also make sure that you see wp_head() in your header.php file before the closing </head> tag.
