1. Installed plugin as an upgrade; problem immediately happened
2. Deactivated; deleted; reinstalled plugin; no change
3. Deactivated; deleted; looked for /wp-content/plugins/slider-captcha/ directory but it did not exist (I was going to delete it if it did exist) reinstalled plugin; no change
