Soft Gated Site – Force form submission before granted access

Hello. I am creating a site that contains a media kit. We want to be able to capture a name and email address before allowing the user to view the site. I don’t really want anyone to have to fully register. I just want a modal to popup when you enter the site that forces you to fill out a form with a name and email, and then you are granted access to the site.

Is there a plugin out there that can do what I want without any extensive customization?
