Some usability suggestions

Hi, first I want to thank you for the hard work on this plug in.

It would be great if you would consider adding these options:

– disable avatar : so if someone is not using avatars the default avatar image and the avatar divs aren’t loaded

– use custom css/disable css : so one can easily use their own stylesheet (without having to deque the files in php)

– disable top comments title (like “8 comments on Post Name”)
