Sometimes Google indexes the posts with /p?= ignoring the permalink

Hello,
My web site is Ruchirablog and I have switched to WP Seo plugin about 2 months ago from all in one seo and so far it feels great.
However Im having a problem which is that sometimes google indexes my posts with the /p?= link ignoring the permalink of the post. And this started to happen after the switch. I didnt had this problem before.And sometimes it indexes the both permalink and /?p= , I guess that’s bad for SEO. And those posts rank well below on google even when searching with the exact title. When I edit those posts I can see that permalink is mentioned on the plugin.
When I’m adding new posts, WP SEO doesnt update the permalink on its box most of the time. It displays /p?= link on the box under editor box. I have to click “save draft” button to get the permalink updated there. Above mentioned blog is running on nginx but I have a small blog on Apache server which has the same problem mentioned on this paragraph.
So what could be the problem here? Appreciate your help ??
