Sort Uploads into Different Folders Based on File Type

Hopefully the title explains it all, but I was wondering if anyone else has explored the possibility of directing uploads into different folders based on their file types.

Right now I have:

wp-content/forms/

When a staff member uploads a pdf or a word doc into the Media Library, I want it to go into wp-content/forms/

But I’d like for photos (screenshots, jpg’s, etc) to all get sorted into the date based setup, preferably under wp-content/uploads/2009/02… etc.

I’ll be looking into this, so if anyone has done it or has pointers or ideas, please let me know. later
