Source Code Button Not Showing

I also have this problem, no matter the placement the icon doesn’t appear.

Unfortunately I have also encountered this same issue after updating the plugin.

Confirmed that it doesn’t show in some cases. Fix coming up in the next version, most likely right after WordPress 4.7 is released.
