St Category Email Subscribe breaks jazzy forms

Hello,
I have just found that St Category Email Subscribe ver 0.6 (from about a month ago) breaks the drop down menu’s on Jazzy forms.
Hope this helps somebody figure out why their jazzy form might have stopped working.
