still can not add form fields

  • I am unable to add any form fields to a project. As soon as i click on the Add new formfield link i see a duplicate title bar with ID, Label, Type, Startpage,Profile, Order, and Order By. But nothing else. No field to input data. I have been ready any post related and nothing suggested helps.
