Still trying to troubleshoot, but not working

Not certain if 4.0.1 update broke it, or a conflicting plugin, but SlickQuiz is not functioning properly right now. Shows field to enter name, but then nothing after clicking button. Ideas? Suggestions?
If I set “Skip the “Start” button?” to yes, it will function.
