I see that you’re using the NUVO theme. It looks like this is a theme issue. When clicking on the booking form in the hero element, it cycles to the next slide.

Unfortunately, NUVO has completely overridden the booking form in this plugin and replaced it with one of its own. I’ve had a lot of people report problems with this, but there’s nothing I can do about it since it’s not my code.

I’d recommend you follow-up with the theme author and ask them for help getting your booking form working.
