[Custom Contact Forms] Stopped working, no error

  • Custom contact form stopped working, I have no idea when; it wasn’t until someone reported being unable to send the form that I noticed. No error, it just reloads the page to a blank form. The form is here:

    I’m a loss at what to look for as there isn’t any errors. I don’t think there were any changes from the time it worked to stopping, but since I don’t know when it stopped working, I can’t say for sure.
