Strange issue with filenames when saving post

I faced an issue with filenames as long as the option ‘Rename On Save’ is on.
Once uploaded the file from the Media uploader on the edit post screen, the file saved as its name, lets say file.jpg. Then, no matter if I change the image title to something else (like ‘New Name’) or keep it as it was initially (file.jpg). But after I update the post the first time, the filename changed to file-2.jpg (adding suffix ‘-2’). When I update the post once again the second time, the filename will change correctly as per image title. if I didn’t change the image title, the filename will remain as file-2.jpg
The same happens from the Media Library.
