    Hey there!

    These are 2 separate issues.

    The issue with the icons is a theme issue, you would want to check with your theme developers about some CSS to change that.

    The issue with the fields not showing is that your site is not using an SSL certificate. You will need to have an SSL certificate installed on your site, and active on your checkout. This will allow the plugin to work in live mode.
