    Hi @rickp01

    The card form rendered for me when testing the page you linked to. You do have a JS error on your page though:

    caught ReferenceError: jQuery is not defined
    at www.cookhamabbey.org.uk/:95:1

    You will want to resolve that error to ensure it doesn’t cause issues with other code. Usually jQuery errors like that are the result of a theme/plugin that is removing jQuery.
