Stripe Setup but no credit card field shows

Stripe is setup on our staging site. The Name field shows during checkout, but not the credit card field. https://www.screencast.com/t/q41sFeDWSW I see it working fine on your demo page https://addons.wp-base.com/payment-gateways/ What am I missing?
