Style.css is merged into Index !!!!

    suddenly and i don’t know why
    Style.css is merged into index.php

    whenever i try google chrome inspect element i get style as Index:13
    and no links to style.css and page source also have the same issue

    Any help to restore Style.css as individual Element ?!!
