Submissable PDF Form Plugin

I have a client who has a number of PDF fillable forms. I’d like a plugin that will allow the user to launch and complete the PDF form, digitally sign it then submit. I’d like it to populate a database as Gravity Forms or Filebase might do.

Any suggestions on a plugin? Thanks
