submission saving and form action errors

I reset some of the form information and updated it- now the submissions are not saving.
Also, I set the form action to go to the homepage, but instead this url comes up and I get a blank page
I didn’t have any problems with submissions before but I haven’t been able to get the form action to work yet at all. (Aside from these issues, it’s been very easy to use- quite a relief.)
