    Hi evanish,

    This depends more on the form / plugin you used to render the form than on the Scroll Triggered Boxes plugin itself. How are you rendering the form?

    If you’re using MailChimp, I highly suggest using the MailChimp for WordPress plugin. To render a form in your box, just use the [mc4wp_form] shortcode and the two plugins will take care of the rest.
