Submit button not working, errors with shortcode

Hey there,
Thank you for creating this plugin especially during these times!

Shortcode doesn’t work for me hen applied it causes critical error.
Using the widget with site origin it doesn’t cause an error but the submit button doesn’t work. I’m running Ascend Premium theme.

Is there a fix for this?
