    Hi Tim,

    Do you have a live URL I can take a look at? Try submitting the form while being logged in as an administrator, you should get a detailed error message telling you what went wrong.

    Most likely, a required list field is missing from your form.

    On a side note, you are a premium users so you can email me using the support email provided from within the plugin to guarantee a faster response. ??
