Submit form, and display submitted data

Hi, Is there any way on the free or Pro version to do a Form that works as “extended profile” i want to do a page where my users put all their medical info, so once they have answered once the next time they will be “editing” that form, not submitting again..

I couldnt find a way to do this with this plugin. Thanks.
