Subscribers not being added to the list consistently

2 emails added to the subscribe space yet no evidence of them being added to my subscribe list despite them saying they cannot add their email again since site informs them they have already subscribed…AND the site is appearing in their reader.

Further test done this morning, subsriber successfully added but no email notification any more to me saying they have subscribed !! So what is going on? Have to sort this out quickly,
