Subscription form

Hi there,

I’m looking to create a form for people to complete that will capture their contact information (name, email, comments) but will also add them to the blog’s subscription list. I currently use Jetpack’s blog subscription widget, but I need something that will allow me to create more fields.

Any suggestions?
