Subscription forms Plugin

  • Hello I’m looking for a subscription form plugin for WordPress.

    Free would be best – can pay if it does the job however.

    I need the ability to add rows of a particular item
    Family Member 1 [First Name] [Last Name] [Other Fields] [Type]
    Family Member 2 [First Name] [Last Name] [Other Fields] [Type]
    … [Add]

    With some conditional logic based on the inserted information. Per item preferably.

    i.e. if Family Member selects Type=2 then show a field, otherwise show something else.

    Ive looked at a few so far and nothing looks to have this feature – or clearly lists this?
