"subscription only form" without option box

Hi there! I’m going to use this plugin, my mailing software is ezmlm, and they seems to work fine together.
I’d like to have a “subscription only form” without the option box (the one where you can chose to subscribe or unsubscribe). I just need a simple email field and the submit button. Could you please help me?
Any hint would be very appreciated. Thanks in advance!
