Suggest some corrections

Your plugin is great but I need to add “City” before the city and “Postal Code” before the postal code. I appreciate you if you can add a section in the settings page that adds the desired phrase before the zip code and city.
Also, I appreciate you if you can add a a section where we can add the desired phrase on the top of the label. As in the attached photo
