Survey Layout

I’ve been asked by a client to recreate a Survey Monkey style form using CF7. They would like to have a selection of questions with 5 Radio options (the options would be the same for each). An example of this is here: https://www.surveymonkey.com/s/CD_Customer_Satisfaction_Questionnaire

What is the best way of recreating this, so the options do not appear after each question but at the top of each row?
