Survey plugin that displays all answers publicly

I’d like to have an area at the bottom of a post that’s like a comments area, but requires users to answer 3 questions (long answer and multiple choice), so a plugin is in order.

I looked at survey/quiz plugins, but haven’t found one that publicly displays the answers of all of the survey-takers. I looked at poll plugins, which do display results, but don’t seem to allow for open-ended (text area) questions. Any ideas?

Or you can use contact form DB plugin to collect and post contents of custom form completions (ie FSContactForm).
