Survey that can do multiple pages?

  • I’m looking for a survey plugin with two requirements:

    1) I’d like it to be able to show multiple pages/tabs

    2) I’d like it to list all the pages on the side so the user can see what page their on, how many there are to go, and be able to navigate from page to page without having to go back a bunch of times of they need to jump around.

    Anyone have any suggestions? Preferably something that’s free but I’d consider something that wasn’t if it does exactly what I need.
