theme forms not working

I have tried everything to get forms working in tesseract 2 theme. My application is not showing up and i have racked my brain and scoured the internet to no avail. It may be a problem in the java scrip code of the theme, but i have no idea.

Everyhting else works just dandy except for the form. I have tried them all and non of them will show.
