Text color incorrect for some fields & how to configure PW and other aspects?

Great plugin!
I submitted a premium support request (and paid a donation) over a week ago, and haven’t heard anything yet, so I thought I might have better luck here!

I have noticed some issues that I would love to get resolved.
When I use certain field types, the color of the text for my labels shows the correct color that I had put into the CSS style for form input field labels, which is white. However, certain other field types show as black (which is undesirable).

The following field types show correctly: text, textarea, email, time, date, URL, and password.

The following field types show the labels’ text color as black, (or as otherwise marked): checkbox, checkbox multiple, radio doesn’t even show ANY text, select- no text, Hidden- no text showing

fieldset(box-open) shows text in blue, no asterisk
fieldset(box-close) shows nothing at all.

I also am curious as to how to configure such aspects as the Radio settings, multiple checkboxes, and password.
I looked deeply into the WordPress Forum for this plugn, and did a Google search as well, but to no avail. I could find no documentation to teach me how to configure these options.

Also, I tried putting Javascript into the HTML after Form Field to have a window popup with the Terms of Service, but while it worked, IE8 showed an “Error on page” alert on the bottom of the browser window.

Can you help me please?
