textbox button and redirect

i am using wordpress now.
i am looking for a code which allow buyer to key in the tracking code inside the textbox, then click on “check” button will redirect to courier tracking link.
For example. my courier tracking link format is as below
i like to do a form when customer key in XXXXXXXXXXXX and click check button, it will auto redirect to https://www.courierworld.com/scripts/webcourier1.dll/TrackingResultwoheader?nid=1&uffid=&type=4&hawbno=XXXXXXXXXXXX
May I know how to make it?
